When you submit a program registration form (such as Refuel & Rewire), Lifestyle Health Initiative (LHI) collects the contact details you provide — your name, email address, phone number, preferred contact time, and how you heard about the program. We also collect limited eligibility information you choose to share, such as residency and coverage status, and any attribution information (such as the referring page or campaign) that helps us understand how visitors find our programs.
We do not request and you should not submit sensitive medical information (such as health-card numbers) through our public registration forms.
We collect this information to review your registration, confirm eligibility, contact you about next steps and participation details, and coordinate the program you expressed interest in. Attribution information helps us understand which outreach efforts are reaching our community so we can improve them.
Your registration information is used to process your program registration, communicate with you about the program, and coordinate participation. With your separate marketing consent, we may also send you future program and wellness updates. You can withdraw marketing consent at any time by replying to any message we send or contacting us at the address below.
LHI does not sell your personal information to anyone, and we do not share it for advertising purposes.
Access to registration information is limited to authorized LHI personnel who need it to administer programs and respond to registrations. We take reasonable measures to protect your information against unauthorized access.
You may request access to, correction of, or deletion of your personal information by contacting us at the address below. We will respond to verified requests within a reasonable timeframe.
Submitting a program-registration form does not create a physician-patient relationship and does not replace emergency or primary medical care. If you are experiencing a medical emergency, contact your local emergency services. Our programs provide health education and do not substitute for individualized medical advice, diagnosis, or treatment from a qualified healthcare provider.
